Page Settings

This is the standard OSX Page Settings dialog.

Here is how it looks when you've selected a specific printer to format for:

The Paper Size menu lets you select create a custom size:

Applications can add a custom page to the dialog (the Settings dropdown is really a tab selector). These pages are from word:

(The last one actually looks like a standard page, but I haven't seen it in other apps)

Print Dialog

This is the standard print dialog with all the standard pages:

Printer drivers can add extra pages to this dialog, and for postscript printers an extra page is generated from the UI description in the PPD file.

Print preview is done by creating a pdf and launching the "preview" app which is the OSX pdf viewer. (Its in a special mode that adds a few buttons)

Easy print dialog

iPhoto added a new "easy print" dialog (availible to all apps) where you can select from a bunch of pre-arranged "styles", each with a bunch of settings. If you click on the Advanced button you get the standard Print dialog.

Illustrator

Illustrator has a completely custom Print dialog (the same dialog is used for Page Setup):

Quark

Quark has a custom print dialog too. The "Page Setup" and "Printer" dialogs bring up a warning about how the system dialogs aren't good enough, and then (on my system) fail to actually open when when you press continue.
